Abstract

A regional protocol has been developed for the management of interactions with coumarins. After two years the participation of pharmacists and the clinical relevance of the interactions have been evaluated. In two years 160 interactions have been reported by 15 out of 19 pharmacists. In 68% of the interactions the interacting drugs were prescribed by different prescribers. 25% of all reports concerned a clinically relevant interaction, i.e. a deviation in INR of more than two units. It is concluded that the central role of pharmacists, due to the diversity of prescribers, and a protocol for reporting interactions contribute to the quality of care.
