Abstract

Not every child responds equally well to asthma maintenance treatment. When a child does not respond well to a low dose of inhalation corticosteroids, the child will be treated with a higher dose of inhalation corticosteroids, or a long-acting beta-2-agonist (LABA) can be added to the existing therapy. However, some children benefit more from LABA treatment than others. This variation can be partially explained by a polymorphism in the gene that encodes the beta-2-receptor (the ADRB2 gene). The working mechanism of LABA depends on this receptor. It is possible to genotype this variant in the hospital. In June 2018, a Dutch, multicenter, randomised controlled trial (the PUFFIN trial) for children with asthma has started to investigate whether ADRB2 genotype guided treatment leads to better control of asthma and whether it is cost effective.
