Abstract

The medication distribution process in the Maastricht University Hospital has been described by flowcharts. Using 'failure mode and effect analysis' as a method for risk analysis, each single part of the process with its potential error has been determined, quantified and ranked. The parts with a high risk score are the prime leads to make improvements in the medication distribution process.
