Optimalisering van de zorg aan CVA-patiënten in de huisartsenpraktijk

As a consequence of the ageing of the population and the nationwide implementation of stroke services, GPs will have to take responsibility for an increasing number of stroke patients. In the near future there is the threat of an expanding gap between supply and demand in the health care of stroke patients. To ensure care in the long term, general practitioners could substantially contribute to optimising care across the whole chain of health care: effective prevention of cardiovascular risk factors based on multidisciplinary guidelines and more attention to patients' adherence to lifestyle advice and preventive medication; improvement of coordination in acute health care; further delegation of chronic care tasks to the nurse practitioner; adequate support for informal carers. Targeting the practice and organisation of GP care on meeting the increasing demands of stroke patients and their informal carers, will also result in a more pronounced and directing position of general practitioners in the care of stroke patients living at home.
