Background: Authors presented in study own modification of surgical approach in treatment of prosthetic grafts infection. The replacement of infected prosthetic graft, most often bifurcated prosthesis, is a high risk surgical procedure, particularly difficult in aorto-duodenal fistula. Material and Methods: This study consist of 37 patients operated for infection of vascular prosthesis. The first group (7 patients) with midline approach and the second group (30 patients) with own modification of approach. An oblique incision from the left costal arch to the right antero-superior iliac spine has been used and the aorta on the level of the renal arteries origin is dissected. Results: In second group shortening of time of operation and decrease of loss of blood was acquired. Conclusions: Own modification of surgical approach makes easier the replacement of infected prosthetic graft and possible GI surgery.
